Enhanced Geriatric Care ManagementGeriatric Care Managers are professional advocates who specialize in assisting elders and their families with long-term care issues. They can help you with one aspect of your homecare needs or take responsibility for setting up every facet of your loved one’s care. The job of a geriatric care manager is to create a care plan that takes into account healthcare, quality-of-life issues, and financial status. A geriatric care manager can serve as a guide through the maze of long-term care services and providers and can offer you the benefit of expertise that can only come with years of experience. Our Enhanced Geriatric Care Management program is staffed by credentialed professional geriatric care managers who provide private, individualized, customized consultation and follow-up on a fee-for-service basis. For more information, or to schedule a consultation, please contact Lucille H. Deutsch, CALA, Director of Geriatric and Caregiver Outreach, at (973) 538-5260, or via e-mail at ldeutsch@fsmc.org.
